The Role of Cytoglobin and Neuroglobin expression in congenital ureteropelvic junction obstruction etiopathogenesis

Aim: Congenital ureteropelvic junction obstruction (UPJ) obstruction is the most common cause of neonatal hydronephrosis. There exist 2 commonly proposed etiologies of ureteropelvic junction obstruction-intrinsic and extrinsic obstruction. Histologic studies in intrinsic obstruction have postulated that obstruction is caused by aperistaltic segments with abnormal amounts of muscle and collagen deposition. Although many studies investigating the molecular changes of this obstructed segment have been performed, the underlying mechanisms are still unclear. Cytoglobin (Cygb) and neuroglobin (Ngb) are recently identified globin molecules that are expressed under hypoxic and/or ischemic conditions. This upregulation suggests possible protective roles through prevention of oxidative damage. Here we aimed to examine the expression level in UPJ obstructions and role of Cygb and Ngb in congenital UPJ etiopathogenesis.

 Methods: Specimens were obtained from 15 children with a mean age of 5 months undergoing dismembered pyeloplasty. Control samples included 5 archival specimens from age-matched children without any history of urologic disease. Paraffin-embedded sections were immunostained to detect Cygb, Ngb, and ki67. Additionally thickness of ureteral wall was determined. Differences between the 2 groups were examined statistically.

 Results: The mean age in UPJ obstructed group was 5 month (3-36 month) and 7 month (5-48 month) in normal group. The mean separated renal function in affected kidney was 35% and mean anteroposterior diameter of renal pelvis was 29 mm. Histopathologically Cygb, Ngb and ki67 expressions were found to be higher in obstructed specimens. (p<0,05) Additionally, the mean thickness of ureteral wall was higher than in controls.(p<0,05)

 Conclusion: Increased expression of CYGB and NGB in narrow segment suggests that hypoxia induced by some conditions, such as vascular accident, can be a reason for intrinsic type UPJ obstruction.

Pelviüreterik Darlıkların Etyopatogenezinde Cytoglobin ve Neuroglobinin Rolü

Amaç: Pelviüreterik bileşke  tıkanıklıkları, (PUBD) hidronefroz nedenleri arasında  ilk sırada yer alır. PUBD, ureterin kendisinden  veya lumen dışından kaynaklanan nedenlerden olabilir. En sık görülen etken ureter lumenindeki fibrotik daralma olup düz kas hücresinde azalma ve kollajen artışı ile karakterizedir. Bölgesel inervasyon bozukluğunun  bu durumdan sorumlu tutulabileceği bildirilmiştir. Neuroglobin ve cytoglobin,  iki yeni  globin olup, dokunun hipoksiye ve iskemi-reperfüzyon hasarına maruz kaldığı durumlarda arttığı gösterilmiştir. Çalışmamızda PUBD olgularında dar segmentlerde neuroglobin ve cytoglobin ekspresyonunun düzeyini incelemeyi amaçladık.

Gereç ve Yöntem: Çalışmada PUBD nedeni ile opere olan 15 olgudan alınan PUB segmenti ile böbrek hastalığı olmayan travma vb  nedenlerle nefrektomi yapılan 5 olgudan alınan PUB segmenti immünohistokimyasal boyama yöntemi ile incelendi. Örneklerde cytoglobin, neuroglobin ve ki67 ekspresyonu değerlendirildi. Ayrıca üreter duvar kalınlıkları karşılaştırıldı.

Bulgular: PUBD grubunda  yaş ortalaması 5 ay (3-36 ay) iken normal grupta 7 ay (5-48) idi. PUBD grubunda etkilenen böbrekte separe böbrek fonksiyonu ortalama % 35 (%16-%45), pelvis ön-arka çap ortalama 29 mm (17-54 mm) idi. Histopatolojik incelemede PUBD grubunda cytoglobin, neuroglobin ve ki67 ekspresyonunun ve üreter duvar kalınlığının normal gruba göre istatistiksel olarak anlamlı düzeyde arttığı görüldü.(p<0,05)

Sonuç: Dar segmentlerde neuroglobin ve cytoglobin ekspresyonunun artışı buradaki darlığın nedeni olarak hipoksiye neden olabilecek vasküler patolojilerin sorumlu tutulabileceğini düşündürmüştür.
