Introduction:Duplication of urethra is a rare congenital urinary tract anomaly. The different anatomical variants of urethral duplication are grouped in three types using the Effmann classification (Effmann et al. Radiology 1976;119:179-85). Type IIB is a rare type in which two urethras arising from the bladder or posterior urethra unit into a common channel distally. We presented transurethral urethral unroofing of a case with type IIB urethral duplication with no other associated anomalies.

Material-method: A 5 years-old male was referred with the complaint of poor urinary stream. Micturating cystourethrogram demonstrated a ventral urethral duplication, arising in the sagittal plane from the bladder neck, blind- ending at the level of the bulbous urethra, compressing the native urethra and causing poor urinary stream. Ultrasonography ruled out associated upper urinary tract anomalies. Urethrocystoscopy showed the normal caliber of the native urethra, with a pin- point communication to ventral duplicated urethra at the bulbous urethra and a dilated opening to the posterior urethra with the help of a guide–wire. The septum from distal opening to come to near to posterior urethra was divided by using the Bugbee hook.

Result: After this simple procedure the patient was relieved of his obstructive symptoms. Postoperative micturating cystourethrogram revealed improvement of the compression of the ventral urethra.  

Conclusion:Urethral duplication has a variable clinical presentation depending upon the urethral anatomy. Surgical management thus will vary from case to case. Symptomatic patients relief can be obtained following simple unroofing of the duplicated urethra. 

Giriş:Üretra duplikasyonu nadir bir konjenital üriner system anomalisidir. Üretral duplikasyonun farklı varyantları Effmann sınıflamasına (Effmann ve ark. Radiology 1976;119:179-85)göre 3 tipe ayrılır. Tip II B, mesane veya proksimal üretradan ayrılan iki kanalın distalde birleşerek tek kanal oluşturduğu nadir bir tiptir. Burada ek bir anomalisi olmayan tip II B üretral duplikasyonda transüretral yoldan üretral unroofing yapılan bir vakayı sunduk.

Gereç ve Yöntem:5 yaşında bir erkek hasta idrar yapmakta zorlanma ve zayıf idrar çıkışı şikayeti ile başvurdu. İşeme sistoüretrografisinde mesane boynundan çıkan ve bulboz üretra seviyesinde kör sonlanan duplike üretral kanalın asıl üretraya basısı nedeni ile zayıf idrar akımı olduğu görüldü. Ultrasonografi ile ek üst üriner sistem anomalileri ekarte edildi. Üretrosistoskopi ile asıl üretranın normal kalibrasyonda olduğu, bulboz üretra seviyesinde iğne ucu tarzında bileşke ile beraber posterior üretrada genişlemiş açıklığı klavuz tel yardımı ile görüldü. Distal bileşkeden posterior üretraya yakın seviyeye kadar septum, Bugbee kanca kullanılarak ayrıldı.

Sonuç:Bu işlemden sonra hastanın obstrüktif belirtileri kayboldu. Postoperatif işeme sistoüretrogramda üretradaki basının düzeldiği görüldü.

Tartışma:Üretral duplikasyonun üretra anatomisine bağlı olarak farklı klinik tabloları vardır. Bu nedenle cerrahi tedavi her vakada farklıdır. Semptomatik hastalarda yalnızca duplike üretranın unroofing’i ile düzelme görülebilir.
