Case Presentation Diagnosed Cloacal Exstrophy

Aim:

Cloacal exstrophy is an extremely rare anomaly seen in 200,000-500,000 live births with severe anomalies such as urogenital, musculoskeletal, intestinal, renal and cardiac in the abdominal wall defects.
Here, we presented our treatment experience in 4 patients who were diagnosed as cloacal exstrophy.

 

Cases:

 Three of our patients are girls and one is a boy. There were omphalocele, renal pathology, short bowel and meningocele in three cases, severe heart anomaly in one patient, neural tumor in one patient, hypoplasia in the left foot and accessory finger in one patient. Primary surgical repair was performed after 2-3 on the day births of our patients. Two omphalocele defects were closed with primer and the other two were covered with a dakron vascular graft.
One patient's pelvic bones were approximated by osteotomy. One patient died on the second postoperative day due to severe cardiac anomalies. At 1 year old male patient, epispadias and scrotal plasty were done. In two patients vesicoureteral reflux developed and sting was performed. All of our patients had connected cord in lumbosacral MR examinations. Our patients were followed up as multidisciplinary management.

 

Discussion:

 Cloacal exstrophy defined as an urogenital membrane is thought to be a result of cloacal membrane rupture before the rectum, without separating the urogenital sinus at the front, and beginning mesenchymal migration from the yolk sac. The ileocecal region in the middle, the bladder halfs in the beside, the omphalose, the symphysis pubis at the top, as well as genital, neural, cardiac and intestinal anomalies coexist. No accompanying anomalies. We think that it is appropriate to treat and follow with multidisciplinary approach after surgical repair of the primary. Because these patients are accompanied by short bowel and genital problems, we find it more appropriate to follow with ostomy.

 

Ekstrofi Kloaka Tanılı Hasta Sunumu

Amaç:

Ekstrofi kloaka karın ön duvarı patolojileri içerisinde ürogenital, muskuloskleteral, intestinal, renal ve kardiyak gibi ağır defektlerle seyreden 200000-500000  canlı doğumda bir görülen oldukça nadir bir anomalidir. Burada ekstrofi kloaka tanısı alan 4 hastamızdaki tedavi deneyimimizi sunduk.

 Olgular:

Hastalarımızın üç tanesi kız, biri erkekti. Hepsinde omfalosel, renal patoloji, üç tanesinde kısa barsak ve meningosel, bir  hastamızda ciddi kalp anomalisi, bir hastamızda nöral tümör, bir hastamızda sol ayakta hipoplazi ve aksesuar parmak mevcuttu. Hastalarımız doğumlarının 2-3. gününde primer cerrahi onarım yapıldı. İki tanesinde omfalosel defektleri primer, diğer ikisinde dakron damar grefti ile kapatıldı. Bir hastamızın pelvis kemikleri osteotomi ile yaklaştırıldı. Bir hastamız postoperatif ikinci günde ciddi kardiyak anomali nedeni ile kaybedildi. Erkek hastamıza 1 yaşında epispadias ve skrotal plasti yapıldı. İki hastamızda vezikoüreteral reflü gelişti ve sting yapıldı. Hastalarımızın hepsinde lumbosakral MR tetkiklerinde bağlı kord mevcuttu. Hastalarımızın takibi multidisipliner olarak yapıldı.  

 Tartışma:

Ekstrofi kloaka ürogenital membranın kloakayı arkada rektum, önde ürogenital sinüsu ayırmadan ve yolk kesesinden mezenkimal göçün başlamasından önce kloakal membran rüptürü sonucu oluştuğu düşünülmektedir. Ortada ileoçekal bölge, yanlarda iki mesane yarısı, üstte omfalose, simfiz pubisin ayrık olması ile birlikte genital, nöral, kardiyak ve intestinal anomaliler eşilk etmektedir. Eşlik eden anomaliler bulunmamaktadır. Cerrahi olarak primer onarım sonrası multidisipliner yaklaşımla tedavi ve takip edilmesinin uygun olduğunu düşünmekteyiz. Bu hastaların takiplerinde kısa barsak ve genital sorunlar eşlik ettiği için ostomili takibin daha uygun buluyoruz.
