Did subureteric injection change the treatment indications in vesicoureteral reflux?

Introduction: Subureteric injection usage increased in recent years alongside surgery for the treatment of vesicoureteral reflux (VUR). Injection is used more widespread than surgery preactically because of it is more easy applicable and less invasive. In this study, the datas of patients performed surgery and subureteric injection for VUR in our clinic were compared for indications.

Material and methods: The hospital records of patients, performed surgery or subureteric injection for VUR between 2000-2017 in our clinic, were evaluated retrospectively. The demographic datas, number of urinary track infection, reflux degrees, presence of scare on kidney, surgey and subureteric injection treatment were recorded. Patients’ voiding cystourethrography and renal scintigraphy were evaluated for reflux degree and scar. Results were compared statistically. 

Results: 128 patients (181 renal units) were treated with antireflux prosedures because of VUR. Famale/male ratio was 68/60 and the average age was 66.1±39.9 months. Antireflux surgery was performed to 87 patients (49 unilateral, 38 bilateral), meanwhile subureteric injection was applied to 41 patients (26 unilateral, 15 bilateral). Both of two groups’ average age, number of urinary track infection, number of treated renal unit, VUR degree distribution and number of scarred renal unit were presented in Table 1. There was not any statistical difference between two groups data, meanwhile only number of renal units with 3rd degree reflux was significant more as statistical in the surgery group than injection group.

Conclusion: Altought there is not any statistically significant between two groups data in our study , we think that to take decision for subureteric enjection is especially more easy for reflux treatment. Subureteric injection especially can be used for low-grade degree reflux as a first choice. The surgery still preserving its importance for the patients injection failed, also high-grade reflux and scarred kidneys.

Vezikoüreteral reflüde subüreterik enjeksiyon tedavi endikasyonlarını değiştirdi mi?

Giriş: Vezikoüreteral reflü (VUR) tedavisinde, son yıllarda cerrahi tedavinin yanında subüreterik enjeksiyon kullanımı giderek artmıştır. Pratikte, enjeksiyonun daha kolay uygulanabilir ve daha az invaziv olmasından dolayı cerrahiye göre daha yaygın olarak uygulandığını düşünmekteyiz. Bu amaçla kliniğimizde VUR nedeniyle cerrahi ve subüreterik enjeksiyon uygulanan hastalara ait veriler endikasyonları açısından karşılaştırılmıştır.

Materyal ve metod: Kliniğimizde 2000-2017 yılları arasında VUR nedeniyle antireflü cerrahi ya da subüreterik enjeksiyon uygulanan olguların kayıtları geriye yönelik olarak değerlendirildi. Bu hastaların demografik verileri ile üriner sistem enfeksiyon sayıları, reflü dereceleri, böbreklerdeki skar varlığı, uygulanan cerrahi ve subüreterik enjeksiyon tedavileri kaydedildi. Hastalara ait miksiyon sistoüretrografiler reflü dereceleri açısından, böbrek sintigrafileri de skar açısından değerlendirildi. Sonuçlar istatistiksel olarak karşılaştırıldı.

Bulgular: Kliniğimizde 128 olguya (181 böbrek ünitesi) VUR nedeniyle antireflü tedavi uygulandı. Kız/erkek oranı 68/60, yaş ortalaması 66,1±39,9 aydı. 87 olguya antireflü cerrahi tedavi (49 hastada tek taraflı, 38 hastada çift taraflı) uygulanırken 41 olguya da subüreterik enjeksiyon (26 hastada tek taraflı, 15 hastada çift taraflı) uygulandı. Her iki grubun yaş ortalaması, üriner sistem enfeksiyon sayısı, tedavi uygulanan böbrek ünitesi sayısı, VUR derecelerinin dağılımı ile skar gelişen böbrek sayısı Tablo 1’ de verilmiştir. Her iki grubun verilerinin istatistiksel değerlendirmesinde, arasında anlamlı fark bulunmazken, sadece 3. derece reflülü böbrek ünitesi sayısının, cerrahi tedavi uygulanan grupta anlamlı olarak daha yüksek olduğu saptandı.

Sonuç: Yaptığımız çalışmada her iki grup arasındaki veriler arasında anlamlı bir fark olmasa da reflü tedavisinde, subüreterik enjeksiyon kararının daha kolay alındığını düşünmekteyiz. VUR’ da subüreterik enjeksiyon, özellikle düşük dereceli reflülerde ilk seçenek olarak kullanılabilir. Bunun başarısız olduğu olgular ile yüksek dereceli reflülerde ve skarlı böbreklerde cerrahi tedavi önemini halen korumaktadır.
