Preoperative and Postoperative Characteristics and Results of Patients With Posterior Urethral Valve

Purpose

To evaluate the preoperative and postoperative findings of  patients operated for posterior urethral valve (PUV).

Material and Method

Thirty five patients operated for PUV between 2008 and 2016 (mean 94 days, range 4 days-12 years). Four had prenatal diagnosis. Preoperative and postoperative VCUG, DMSA, creatinine and story of  urinary tract infection are sought. Additional surgical procedures are also evaluated.

Results

No VUR was demonstrated in 4 patients. No VCUG is obtained in 2 patients (Table 1). Most of the bilateral VUR cases were grade IV or more. Twelve patients had a history of urinary tract infection. Thirteen patients had significant neuropathic  bladder findings on VCUG and 21 had dilatation of posterior urethra.

Preoperative/postoperative creatinine 0,94 (0,14-6,63)/0,39 (0,11-2,3) (mean-range). Five on CIC (3 through Mitrofanoff conduit). Two received augmentation+Mitrofanoff and 1 Mitrofanoff only. One underwent bilateral ureterostomy and 1 unilateral ureterostomy (undiversion at 5 years).  Three received percutaneous vesicostomy catheter and 1 diverged to permanent vesicostomy. Seven has ESRD and 2 permanent peritoneal dialysis catheters are placed. Postoperative UTI encountered in 22. Forty-two sessions of subureteric injection (17 right, 14 left, 11 bilateral). Urodynamic study conducted on 27 patients; 14 diagnosed as neuropathic bladder and 9 had DSD. Low capacity and compliance in 17.

Conclusion

PUV is chronic disease with low chance of cure in most of the patients and necessitate long term follow-up.

Posterior Üretral Valv Tanılı Hastaların Operasyon Öncesi ve Sonrası Takip ve Sonuçları

Amaç: Posterior Üretral Valv (PUV) nedeniyle opere olan hastalarımızın operasyon öncesi ve sonrası değerlendirilmesi

Hastalar ve Method:2008-2016 tarihleri arasında PUV kesisi yapılmış 35 hasta ( ortalama 94 gün; en küçük 4 gün-12 yaş) çalışmaya alındı. Dört hastada  antenatal tanı mevcuttu. Hastaların operasyon öncesi ve sonrası VCUG, DMSA, serum kreatinin ve İYE mevcudiyeti değerlendirildi. Ayrıca ek cerrahi girişimler belirlendi.

Bulgular

Dört hastada VCUG’de VUR görülmedi. İki hastanın preoperatif VCUG’si yok (Tablo 1). Çift taraflı VUR’ların çoğu derece IV veya derece V. Preoperatif İYE geçiren hasta sayısı 12. Onüç hastada belirgin nöropatik mesane görüntüsü mevcut olup posterior üretral genişlemenin belirgin olduğu hasta sayısı 21.

Preoperatif serum kreatinin; ortalama;  0.94 mg/dl (0.14-4.63) ve postoperatif serum kreatinin; ortalama; 0.39 mg/dl (0.11-2.3). Beş hasta TAK yapıyor (3’ü mitrofanofftan). İki hastaya ogmentasyon/mitrofanoff ve bir hastaya sadece mitrofanoff uygulandı. Bir hastada bilateral üreterostomi ve bir hastada sol üreterostomi açıldı (5 yaşında kapatıldı). Üç hastaya sistofiks takıldı ve biri vezikostomiye çevrildi. Toplam yedi hastada KRY tanısı mevcut ve ikisine kalıcı periton diyalizi takıldı. Yirmiiki hastanın postoperatif İYE öyküsü mevcut. Kırkiki adet enjeksiyon yapılmış (sağ 17, sol 14, bilateral 11). Yirmiyedi hastanın ürodinamisi değerlendirildiğinde; 14 hasta nöropatik mesane tanısı almış olup 9 hastada DSD mevcuttu. Onyedi hasta düşük kapasiteli ve kompliyanslı olarak değerlendirilmiş.

Sonuç

Posterior ürethral valv, sonuçları açısından bakıldığında kalıcı tedavisi birçok çocukta oldukça güç bir hastalıktır.
