Is Surgical Outcome of Megameatus Intact Preputium Worse than Distal Hypospadias?

Aim:

Megameatus intact preputium (MIP) is often perceived as a simple defect because of the distal location of meatus and the width of the urethral plate that can easily be tubularized. This study aims to evaluate the outcome of cases who underwent surgery for MIP.

Patients - Methods:

Hospital records of patients who operated for MIP and coronal hypospadias between years 2007-2015 were reviewed retrospectively. Demographics of patients, surgical details, and postoperative complications were compared between groups.

Results:

There were 22 patients with MIP and 29 patients with coronal hypospadias with median ages of 36 months (1-180) and 48 months (7-126) respectively. The meatus was glanuler in 23, coronal in 8, subcoronal in 2 patients in MIP group. There were 3 patients with chordee in coronal hypospadias group. Urethrocutaneous fistula developed in 6 patients (%18) who underwent surgery for MIP. Two of these cases underwent Pyramid, one of them Mathiue urethroplasty the other three Duplay urethroplasty. Urethrocutaneous fistula developed in one (%3) patient with coronal hypospadias. Snodgrass urethroplasty was performed in all of the patients in this group. The difference of fistula rate between groups was not found statistically significant due to smallness of the groups (p=0.054).

Comment

Contrary to belief; complications are not infrequent after megameatus intact preputium despite the applicability of different surgical techniques. This is probably due to the differences in embryogenesis.

Amaç

Hipospadias olgularında anomalinin ağırlığını değerlendirmede mevcut kriterler yetersiz kalmaktadır. Meatus lokalizasyonun distal olması ve rahat tübülarize edilebilecek genişlikte bir üretral plak olması sebebiyle megameatus intakt prepisyum (MIP) çoğunlukla basit bir defekt olarak algılanmaktadır. Bu çalışmada kliniğimizde MIP nedeni ile opere edilmiş olguların sonuçlarının komplikasyonlar açısından değerlendirilmesi amaçlanmıştır.

Hastalar-Yöntem

2007-2015 yılları arasında kliniğimizde MIP ve koronal hipospadias nedeni ile opere edilen olguların dosyaları geriye dönük taranarak demografik özellikleri, operasyon detayları ve postoperatif sonuçları karşılaştırmalı incelenmiştir.

Bulgular

MIP nedeniyle opere edilen 33 olgu ile koronal hipospadias nedeniyle opere edilen 29 olgunun ortanca operasyon yaşları sırasıyla 36 (1-180) ay ve 48 (7-126) aydır. MIP nedeni ile opere edilen 23 olguda meatus yerleşimi glanüler düzeyde iken sekizinde koronal, iki olguda ise subkoronal yerleşimlidir. Koronal hipospadias grubunda 3 olguda kordi mevcuttur, tamamı deri serbestleştirilmesi ile düzelmiştir. MIP nedeni ile opere edilen 6 (%18) olguda postoperatif fistül gelişmiştir. Bu olgulardan ikisine piramid, birine Mathiue üretroplasti, üçüne ise Duplay üretroplasti uygulanmıştır. Koronal hipospadias grubunda ise 1 (%3) olguda fistül gelişmiştir. Bu grupta tüm olgulara Snodgrass üretroplasti uygulanmıştır. Gruplar arasında fistül gelişimi açısından fark görünse de, grupların küçüklüğü sebebiyle anlamlı istatistiksel fark saptanmamıştır (p=0.054). MIP grubunda bir hastada divertikül gelişmiştir, asemptomatik olduğundan izlenmektedir.

Yorum

Megameatus intakt prepisyum onarımı farklı cerrahi tekniklerin uygulanabilirliğine rağmen öngörülenin aksine sık komplikasyon gelişebilen bir cerrahidir. Bu durum olasılıkla anomalinin embriyolojik farklılığına bağlıdır.
